Scoliosis Imaging is Not Available at Outpatient Locations
If you received a referral for a scoliosis x-ray, please go to Imaging and Radiology at HealthPark Medical Center or Lee Health Coconut Point. Scoliosis x-rays are not available at other Lee Health facilities.
General Patient Instructions
When scheduling appointments, please have the following information available: your physician’s order (prescription), insurance information, and medical history.
Before your arrival to appointment, please bring:
- A copy of your physician’s order (prescription)
- Your insurance card
- A photo ID
