Managing your medications can be overwhelming and complicated. Lee Health makes it easier by offering medication therapy management (MTM).

A Lee Health pharmacist will conduct a one-on-one MTM appointment for any medication-related concerns.

How Does MTM Work?

Lee Health’s medication therapy management pharmacists will:

• Review your medication regimen
• Identify medication-related problems including potential side effects
• Address medication concerns by making recommendations to your providers
• Clarify information and answer your medication questions

Schedule an Appointment

Appointments are conducted in-person at a Lee Pharmacy or over telehealth. Schedule an appointment through MyChart or ask your Lee Physician Group provider to place a referral.
