Cardiac Care

Lee Health's Home Health Services are proud to have been awarded the prestigious Heart Failure Disease Program Certification by the Community Health Accreditation Partner (CHAP). This certification recognizes our commitment to delivering the highest standard of care, adhering to rigorous program evaluations that focus on structure, function, quality metrics, and education.

Our providers are dedicated to providing comprehensive cardiac care including patient assessment, development of a care plan, teaching the patient/caregiver about diseases, and self-care to prevent rehospitalization.

Rehabilitation Services

Rehabilitation plays a vital role in recovery from major orthopedic, neurological, cardio-pulmonary, and general surgeries along with major events such as a stroke.

Licensed physical therapists, speech pathologists, and occupational therapists treat patients in their homes. 

After an evaluation, a therapist designs a unique treatment plan based on the patient's needs.

The patient and caregiver are closely involved in establishing the treatment goals. Most patients use these services after leaving the hospital, a rehabilitation hospital, or skilled nursing facility, but patients also come to us by referrals from a primary care physician.

Infusion Therapy

We also provide patients with a wide range of parenteral/enteral therapies. Access Infusion, Lee Health's infusion therapy provider, works with us to provide your nursing and IV pharmacy needs.

Those needs may include IV antibiotics, pain management, central line and tube feedings, other IV medications, and all of the supplies associated with the delivery of IV therapy.

Ostomy and Wound Care Management

Our team of knowledgeable wound care certified registered nurses act as advisers to the rest of the care providers. We also have experienced ostomy care nurses to promote promote healing and self-care, using an individual approach to the patient's needs.

Diabetes Management

Lee Health’s Home Health Services provides comprehensive diabetes care for patients of all ages. Our nursing staff works with physicians to develop a treatment plan to help meet your specific goals.

Our nursing staff will teach you about medication management, diet, signs/symptoms, monitoring of blood sugar, and actions to take if a problem arises.

Our physical therapists will evaluate your needs and develop a home exercise program to help you maintain a healthier, more active lifestyle that will reduce complications.

Perinatal Services

Our goal is to meet the special health care needs of pregnant women with skill and compassion.

Pregnancy can place a variety of stresses on the body, and your physician may recommend home monitoring in cases of pregnancy-induced high blood pressure, diabetes, wound infections, pre-term labor, pregnancy-induced nausea, or vomiting.

Our services include non-stress testing, Heparin therapy (blood thinning), intravenous hydration and antibiotic therapy, post-partum care, breastfeeding education, lab work, and gestational diabetes education.
