Our Home Health Care Team

Registered Nurse

The registered nurse (RN) often makes the first visit to assess the patient’s health. Registered nurses:

  • Return at regular times to check on your nursing needs
  • Start the plan of care and revise the plan of care as needed
  • Give services that need specialized nursing skills
  • Start suitable nursing actions for prevention and recovery
  • Coordinate services
  • Inform the doctor and other health workers of changes in your condition and needs

Licensed Practical Nurse

The licensed practical nurse (LPN) works with and under the direction of the registered nurse. Licensed practical nurses:

  • Assist the register nurses and doctors in giving care
  • Work together to figure out the best course of care
  • Assess, monitor, and record symptoms, vital signs, and health changes
  • Give direct patient care
  • Perform tests and procedures
  • Educate the patient and their families on the safe treatment of injuries and illnesses

Physical Therapist

The physical therapist (PT) is a health professional who helps ease your pain and help you move better. This therapy helps you carry out life tasks well, and so you can have the best quality of life. The physical therapist:

  • Works to ease pain and stiffness
  • Helps restore movement
  • Evaluates, diagnoses, and treats patients of all ages 

Occupational Therapist

The occupational therapist (OT) is a health professional who uses everyday life activities in healing. The OT will help you change your movements or use adaptive equipment if you have an injury, illness, pain, or a disability that makes it hard for you to do your job or schoolwork, care for yourself, complete household chores, or be active.

Speech Therapist

Speech therapists, also known as speech-language pathologists (SLPs), diagnose and treat patients with swallowing problems, speech problem or language problems. They also treat patients that may have a hard time communicating because of problems with thinking.

Respiratory Therapist

Respiratory therapists (RTs) work closely with patients with lung diseases, such as chronic obstructive pulmonary disease (COPD). RTs teach about the causes and symptoms of their disease. They teach about oxygen safety. They teach about the right ways to take medicines. They teach about the best ways to breathe, about therapy to expand the lungs, and about mucus clearance. They teach about CPAP/BiPAP.

Medical Social Worker

There may be important social or emotional factors related to health problems. Medical social workers help the doctor and other team members figure out these factors. They take part in working out the plan of care. They work with the family. They use suitable support from the public. They help plan for patients to be discharged from Home Health.

Home Health Aide

Home health aides mostly help with personal care and activities of daily living. They work under the direction of a nurse or therapist and:

  • Help with bath or shower
  • Help with skincare, oral hygiene, or hair care (They do not cut hair or shave patient)
  • Help with the preparation of simple meals
  • Help maintain a clean and safe bedside area
  • Help by changing bed linens
  • Help the patient with toileting or catheter care
  • Help the nurse or therapist with teaching programs to aid in rehabilitation
