What is the Heart Care Transition Clinic?

The Heart Care Transition Clinic is a specialized medical office for cardiac patients recently discharged from the hospital to receive follow-up medical care. The Lee Health Heart and Vascular Institute Heart Care Transition team works with your cardiologist to ensure you get the follow-up care you need.

It is important for patients with a diagnosis of heart failure, cardiomyopathy or A-fib to go to a follow-up medical appointment shortly after discharge from the hospital.

The Heart Care Transition Clinic has two locations in Fort Myers and one in Cape Coral. In Fort Myers, you can find us inside the main atrium at HealthPark Medical Center and also conveniently located on Metro Parkway near Gulf Coast Medical Center. Our Cape Coral location is situated near Cape Coral Hospital on SE 8th Terrace. At these clinics, you'll be cared for by a specially trained cardiac nurse practitioner who conducts thorough and personalized visits to address your heart health needs.

What will I experience at the clinic?

You will meet with the Nurse Practitioner who will:  

  • Evaluate your overall condition and response to hospital therapy.
  • Evaluate your medications making adjustments if necessary.
  • Help arrange follow up diagnostic studies related to your medical condition.
  • Provide education for a better understanding of the diagnosis, including symptoms to monitor and who to report any changes to.
  • Assist with arranging follow-up with your cardiology team as well as any other specialist.

How does the Heart Care Transition Clinic work?

The outpatient clinic sees patients three to five days after admission for heart failure. The advanced providers review hospitalization, medications, labs, tests, and notes. Any changes in medications after hospitalization, the providers will talk with the patients and check them at the clinic. After that, you will see your primary cardiology team depending on your visit.

  • Too much alcohol consumption
  • Salt in foods (processed food, fast food, canned food, lunchmeats)
  • Don't take medications as advised
  • Blood pressure too high
  • Heart being too fast (Tachycardia)

What are the symptoms of heart failure?

  • Shortness of breath
  • Weight gain
  • Abdominal bloating
  • Poor appetite
  • Swelling
